RESIDENTIAL CARE
We offer high quality care to anyone over the age of 65, and specialise in care for people with dementia. Residents are accommodated in single rooms (most of which are en-suite) and all being fitted with call bells. Care and Support Plans are tailored to the needs of each individual and reviewed on a monthly basis (or more often, if needed).
Full housekeeping and laundry services are included in the weekly fee, as are all meals, snacks and refreshments. Papers can be ordered from the local newsagent. We have a visiting hairdresser who comes in every Monday, and a chiropodist who calls every six weeks. These additional services are not included in the weekly fee, but are invoiced separately.
At Pilton House we have a dedicated activities team who work 7 days a week. Throughout the day they provide bespoke activities and exercises focusing on the mind, body, and soul. Whilst we have an activities planner, our team will provide either one to one or group activities to those who wish to join in.
At Pilton House we are fortunate enough to have weekly visits from the Older Person’s Integrated Care (OPIC) team. A nurse practitioner visits the home and will discuss any resident that either the team, a resident’s family, or the resident themselves wish to. They discuss any health concerns, either new or ongoing, and will advise appropriately. Once a week the same nurse meets with a multidisciplinary team for the local community and will ask the relevant professional for advice on treatment options or referrals. The Older Person’s Integrated Care (OPIC) team in the United Kingdom is a specialised group of healthcare professionals who provide comprehensive and coordinated care services to residents in care homes. The OPIC team works closely with care home staff, residents, and their families to ensure the well-being, health, and quality of life of older individuals. Here are some key responsibilities and services provided by the OPIC team:
-
Care Planning and Coordination: The OPIC team assesses the individual needs of each resident and develops personalised care plans. They coordinate with care home staff to ensure that the care provided aligns with the residents’ goals and preferences. This includes coordinating medical care, therapy services, and social activities.
-
Health Monitoring and Assessment: The OPIC team regularly monitors and assesses the health status of residents. This may involve regular check-ups, medication management, and coordination with primary care physicians or geriatric specialists. They also provide guidance and support for residents with chronic conditions, helping them manage their health effectively.
-
End-of-Life Care: The OPIC team plays a crucial role in supporting residents and their families during end-of-life care. They provide palliative care services, manage symptoms, offer emotional support, and facilitate discussions about advanced care planning and decision-making.
-
Mental Health and Well-being: The OPIC team addresses the mental health and well-being of residents. They conduct mental health assessments, provide counseling and therapy services, and implement interventions to support residents with dementia, depression, anxiety, or other mental health conditions. They also promote social engagement and activities to enhance overall well-being.
-
Rehabilitation and Therapy Services: The OPIC team includes therapists such as physiotherapists, occupational therapists, and speech and language therapists. They assess residents’ functional abilities, develop personalised therapy plans, and provide rehabilitation services to improve mobility, independence, and communication skills.
-
Falls Prevention and Safety: The OPIC team focuses on preventing falls and ensuring the safety of residents. They assess the care home environment, provide education and training to staff, implement safety protocols, and offer advice on adaptive equipment or modifications to reduce fall risks.
-
Training and Education: The OPIC team provides training and education to care home staff, helping them enhance their knowledge and skills in delivering person-centered care. This may include training on dementia care, communication techniques, infection control, and recognizing signs of abuse or neglect.
-
Care Transitions and Discharge Planning: The OPIC team supports residents during care transitions, such as hospital admissions or discharges. They facilitate communication between care home staff and external healthcare providers, ensuring a smooth transfer of care and continuity of services.
-
Collaboration and Partnership: The OPIC team collaborates with various stakeholders, including care home managers, primary care teams, community healthcare services, social services, and local authorities. They work together to improve care coordination, share information, and implement best practices in older person’s care.
The goal of the OPIC team is to provide person-centered, holistic, and coordinated care to residents in care homes. They strive to enhance the quality of life, promote independence, and ensure that older individuals receive the support they need to age with dignity and well-being.
